Is this about my practice, or the radiologists who report our imaging?
It’s about your practice. This declaration is made by your practice as the contracting business — the legal entity named on this form — and signed by an authorised representative. Where it refers to “an independent business”, that means your practice, not the individual radiologists who report your imaging.
Why am I being asked this now — is it new?
Nothing about your arrangement with Next Healthcare changes. It’s a standard payroll-tax record-keeping step we’re completing with the practices we work with, so our records reflect how each arrangement works.
Some of the details shown are wrong — do I need a new link or a new form?
No. Your link only identifies your practice; it doesn’t lock anything in. Type your correct legal entity name and ABN straight over whatever is shown, then submit — what you enter is what we record. There’s no need to request a new link, a new form, or to cross anything out.
I don’t recognise the provider numbers shown — are they ours?
No, and you don’t need to. They’re the Medicare provider numbers the Next Healthcare radiologists reporting your imaging bill under for your practice — our own reference, not your practitioners’ numbers. There’s nothing to correct or add, and you don’t need to send us your own provider numbers. The declaration itself is about your practice as the contracting business.
